_Resource/PRN Social Worker - Greene Memorial Hospital/Soin Medical Center_
Identification and assessment of patients and families who have social, psychological and/or environmental needs related to the impact of admission, diagnosis, treatment or discharge. Essential duties include, but are not limited to, interviewing patients and families, developing plans for intervention addressing agreed upon priorities, synchronizing discharge of patient with delivery of home services and assisting patients/families in improving or restoring their capacity for social functioning.
Requirements
EDUCATIONAL REQUIREMENTS:
Bachelor of Social Work or Registered Nurse with current Ohio license .
Master of Social Work preferred.
SKILLS AND EXPERIENCE REQUIRED:
Awareness of developmental needs over the age span of infant to elder required.
Ability to work well in multidisciplinary and independent settings.
Ability to communicate verbally and through the use of writing skills.
Ability to coordinate work effectively with other hospital disciplines.
Ability to recognize and understand the dynamics of individuals from diverse populations
Knowledge of and experience with various community resources and post-acute care options
Ability to use a computer and other office equipment requiring finger dexterity, eye/hand coordination, and visual concentration to effectively communicate.
Ability to solve problems through the analysis of information. SKILLS AND EXPERIENCE PREFERRED:
Clinical experience in a health care setting
Planning and delivery of educations session(s)
Precept of new employees
Proficient and comfortable with various psychosocial issues that are present during an individual's hospitalization
