Monday - Friday.
On-call one weekend a month.
Provides and coordinates services including the necessary documentation regarding client care according to the client plan of treatment.
Successfully communicates with the office, physicians and other agencies providing services to a client including other A & D staff such as LPNs and or Home Health Aides.
On-call responsibilities are mandatory.
Requirements:
Positive, interpersonal skills.
Home health care experience required.
A thorough understanding of confidentiality.
A minimum of one year of experience as a Registered Nurse, eligible to practice in the State of Ohio.
Graduate of an accredited School of Nursing.
