Campus OverviewSoin Medical Center Soin Medical Center has been serving residents of Greene, Western Clark, Eastern Montgomery, and Miami counties since 2012.
Conveniently located off I-675 in Beavercreek.
Provides a variety of health care services and is home to 4 accredited Centers of Excellence including hernia and robotic surgery.
Soin is currently licensed for 125 beds which will increase after the expansion completion in the Fall of this year.
In 2020, Soin received an A from the Leapfrog Group, a national patient safety watchdog, ranking among the safest hospitals in the United States.
Accredited by the American College of Emergency Physicians as a Level 3 Geriatric Emergency Department.
We partner with local farms in Greene County to provide fresh produce to employees, patients, and their families.

Patient Safety Excellence Award (2017-2019) Outstanding Patient Experience Award (2017-2019) Department OverviewThe Community Health Worker is a neighborhood-based trusted member of the community and serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services.
The Community Health Worker also strengthens individual and community agency by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, referrals to community resources, social support, and advocacy.
Performs other duties as assigned.
Responsibilities & RequirementsJob ResponsibilitiesAttend specific community meetings to build relationships with those who attend.
Working with program participants, make referrals to community resources to overcome social determinants of health that are adversely affecting them.
Follow-up with these participants to find out if they used the referral resource and/or how it went.
Document these interactions in the program database, if appropriate.
Assist with gathering accurate report information by stated deadlines, as needed.
Attend community events, representing the grant and outreach programs and Kettering Health.
Create and sustain good relationships with community organizations that can provide resources to program participants.
Communicate patient needs with program physicians, as needed.
Maintain adequate records in project software and assist with agency reports, as needed.
Must have reliable transportation Requirements High school diploma or GED required.
Graduate of community health worker training program.
Certified Community Health Worker or obtain certification within 6 months of employment.
Excellent communications and organizational skills.
The ability to work with diverse groups and individuals with dignity and respect, Willing to travel and work throughout green county.
The ability to work varied hours/ some evenings and weekends.
Highly motivated and can work independently.
Able to work collaboratively.
Familiarity with Microsoft Office.
Can lift at least 30 pounds properly.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
