Qualifications:
A Bachelor s degree in Social Work, Counseling, i.
eMental Health, Substance Abuse, or related field, or Master s degree in Social Work, Counseling, or related field.
Two years of experience in chemical dependencyCurrent valid Ohio driver s license & access to a motor vehicleLicensed Social Worker or Professional CounselorRequired to play an active role during a public health emergency, crisis, outbreak, incident, or event in which the Health Commissioner deems additional changes in responsibilities and working locations /hours.
Preferred
Qualifications:
Licensed Chemical Dependency CounselorExperienced in community- based consumer interaction.
Responsibilities include but not limited to:
Providing case management servicesProviding crisis intervention and de-escalation techniques with consumersReferring consumers to community programs/services and conducts follow-up as necessaryPerforming a bio/psycho/social assessment of consumer s mental and physical health, diagnosing, and evaluating their substance abuse treatment needs, situation, concerns, and barriers to accessing servicesFormulating individualized consumers centered treatment plans in collaboration with the consumers and consumer s designated support systemMonitoring, updates and modifies treatment plan as necessary, to be consistent with consumer s needs and/or conditionsPerforming individual, group, and family counselingConducting on site drug testing and interprets urine drug screening results from labCompiling consumer records and maintains documentation according to program and accreditation standards and protocolsMaintaining confidentiality, respect consumer s privacy, routine, and independence as much as possibleDevelop and maintain professional relationships with community partners.
Distribution and oversight of referrals received from community partners.
