The Case Manager is a registered professional nurse with education, knowledge and experience related to the specific unit assigned. The Case Manager's role focuses on leadership in the multidisciplinary planning and coordination of the patient's care across the healthcare continuum. The case manager organizes the sequence of patient care to attain maximum levels of effectiveness and efficiency to reach desired quality, clinical and cost outcomes. The case manager organizes multiple strategies to insure the care of all patients is managed with minimal overlapping roles and processes. The case manager participates in a team approach, composed of the physician, nurse, social service, and other direct and indirect providers and payers.
Current Ohio RN liscensure- BS/BSN required. CCM/ACM preferred. 5 years clinical experience with 2 years case managment preferred, computer and communication skills required. This position is located at Sycamore Medical Center.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
